OC Care Connections is specifically designed for seniors who are frail, require extra care and supervision or have been diagnosed with mild to moderate memory impairment.
Care Connections is available weekdays from 9:30 am - 3:30 pm.
Eligibility will be determined prior to admission.
To Be Eligible For The Program, Participants Must Be:
- Ambulatory
- Continent
- 55 years or older
- Under the care of a physician
- Tested for TB within the past 12 months
- Diagnosed with mild to moderate memory impairment or other related disorders requiring supervision or care.

Family members need respite from the physical and emotional demands of 24-hour care. At the same time, diverse activities, attention and interaction with others are healthy for the elderly. OC Care Connections provides affordable care that meets the functional, creative, emotional, and spiritual needs of the elderly. This program is open to anyone regardless of religious affiliation.
